The initiative provides a single, coordinated system that connects individuals, families, and communities to the right support at the right time—from clinical treatment to preventive wellbeing services.
Developed through insights from Dr. Michelle Lee and team, who identified critical gaps in service navigation and coordination, MeHA Care Connect is committed to improving how care is accessed, delivered, and connected across both clinical and community settings.
